Caring Together

Caring Together was an innovative partnership programme from Marie Curie, the British Heart Foundation and NHS Greater Glasgow and Clyde to improve the quality of palliative and end of life care for patients in the advanced stages of heart failure. 

Launched in March 2011, our partnership programme piloted new integrated care models in Greater Glasgow and Clyde to help people with advanced heart failure live more comfortably with their illness and have a better quality of life. The programme covered a population of 1.2 million people across a diverse geographical area.

The Caring Together programme was independently evaluated in 2016. The findings show that the programme:

  • improved symptoms and quality of life for people with advanced heart failure
  • provided individual patient planning
  • reduced hospital admissions and healthcare costs.
